All youths and adults applying to participate as members of district provisional units (as well as all staff members) at the Mount Vernon encampment must read   and agree to abide by all terms of the following statement. For all Scouts who are less than 18 years old, a parent or guardian must also countersign to indicate that they will support the Scout in meeting all terms of the statement:

u I affirm that I am an registered and active member of the Scouting unit(s) listed above on this application form.
u I affirm that I have completed the requirements associated with Mount Vernon's "200 Years" Scout Salute to George Washington and that I have attached a copy of my ten "Scout Rules of Civility for the 21st Century" and plans for a community event that I will conduct (or have conducted) to commemorate the 1999 Bicentennial of George Washington.
u I affirm that during the first part of June I will pay the full fees required by the council and district to participate as a member of a provisional unit assembled by my district at the Mount Vernon encampment.
u I affirm that I will participate in all planning meetings and "shake down" activities scheduled by the provisional unit of which I am a member in order to ensure that we function effectively as a unit during the Mount Vernon encampment.
u I affirm that I recognize the unique opportunity that is being provided to me to camp and participate in special activities at the Spirit of George Washington Encampment at Mount Vernon on November 5-7, 1999. Because I will be representing all Scouts in the National Capital Area Council, I will behave in an exemplary manner at all times I am on the Mount Vernon grounds. My actions will be guided by the Scout Oath and Scout Law at all times; I will show respect to all people and property; and I will work to ensure that the encampment brings honor to me and all other participants.
